Building
a foundation of trust by treating our patients as special
individuals is vital to our success. We understand how
uneasy some patients may feel about their dental visits,
and how we can make a difference in providing a relaxing
and positive experience. Our entire team is dedicated
to providing you with excellent, personalized care and
service to make your visits as comfortable and pleasant
as possible.
Initial Examination
Your initial examination will involve a comprehensive
examination of your oral condition including radiographs
(x-ray images) as needed. Once all the data is collected
an accurate diagnosis and comprehensive treatment plan
will be presented.
Please assist us by providing the following information
at the time of your consultation:
. Your surgical referral slip and any x-ray images if
applicable
. All the completed forms provided to you prior to your
initial exam
. If you have dental insurance, bring the necessary documentation.
This will save time and allow us to help you process any
claims.
. Please alert the office if you have a medical condition
that may be of concern prior to your visit (i.e. heart
murmur, history of rheumatic fever, diabetes, high blood
pressure or any artificial heart valve or joint) or if
you are on any of the following medication (i.e. heart
medications, coumadin, steroids, aspirin, anticoagulant
therapy, etc.)
