Prior
to your visit at our facility, one of our well-trained
Treatment Coordinators will make arrangements with you.
These coordinators assist you by answering any questions
in regards to our practice, philosophy as well as the
details regarding your first visit. Any relevant patient
forms will be provided to you. This can be done either
through the mail, or may be downloaded using the link
“registration”.
Broken/Missed Appointments
Treatment time is specifically reserved for each individual
patient. In consideration for all our patients with serious
needs for appointments, we request AT LEAST 2 business
days NOTICE if you are unable to keep your appointment.
A missed appointment charge will be assessed to your account
after the second missed or broken appointment. |
